Intern/Clinical Application Form
Note: All interns that work directly with clients or client records must attend a training session provided by Tattoo Art Therapy. Apprenticeships and Intern Clinicals will be conducted and supervised by Eric Deann Spruth, MA, ATR, Certified Laser Technician.
This application must be completed in one session if done online. It can take up to 30 minutes to complete, so please give yourself enough time.
